Medical Director

Physician responsible for developing a comprehensive medical care plan for each patient.

Skilled Nurses

Trained nurses who provide home visits and perform assessments. Nurses create a plan of care with the Medical Director

Home Health Aide

& Homemaker Services

These services are designed to give peace of mind and comfort. They provide direct personal care and support to the patient and family, and report significant clinical changes to the Registered Nurse Case Manager.

Spiritual Counselors

Our non-denominational Chaplain makes initial contact with each patient and family to assess and give spiritual needs.

Trained Volunteers

Hospice Volunteers compassionately give assistance and comfort through friendly visits.

Attending Physician

Primary care physician that works as part of the hospice team.

Medical Social Workers

Assesses social service needs of the patient and family.
Provides counseling, assists in coping with anxiety, fear, stress, depression, family conflicts, financial needs and concerns about death.
They can also provide assistance with funeral and memorial preparations.

Dieticians

Provides nutritional counseling.

Physical | Occupational | Speech Therapist

Services that will promote comfort in a traditional rehabilitative environment.
